Family: Assessment/Case Planning Service

An individualised, dynamic written plan or a support agreement that is developed between family in the program and agency (delivered or resourced by the IFP program) as a result of an assessment process may include;

• Goals and objectives;
• Planned measures/actions;
• Needs assessment (including: health, education, therapeutic social/cultural support);
• Family/relatives/kin contact arrangements;
• Living/accommodation arrangements.
