Definition

The difference (net shift) between the earliest and latest SCORE (circumstances and goals) for the Outcomes Domain under the same Service Types or Program Activities.

A positive shift in outcomes signifies an improvement in outcomes.

Computation Rule
Description

The net shift (difference) is calculated as the latest Outcome Domain Score minus the earliest Outcome Domain Score, disaggregated by Outcome DomainProgram Activity and Service Type for clients who are fully assessed.  For a client to be fully assessed, they require paired AssessmentID from the same ClientIDParticipation typeActivityIdServicetypeId, LeadOrgIdDeliveryOrgIdOutcomeType, and OutcomeDomain. The Paired SCORE require the second SCORE, identified by AssessmentID, to be indicated as AssessmentType = 'Post-SCORE.' Outcome Type must equal "Circumstances" or "Goals". The final Assessed date is used for reporting of the results of the paired assessments.

SCORE stands for ‘Standard Client / Community Outcomes Reporting’. It is an outcomes reporting tool that helps report the impact of service delivery. In the Data Exchange, there are four different types of SCORE:

  • Circumstances SCORE: measures changes in individual client circumstances.
  • Goals SCORE: measures an individual clients progress in achieving specific goals.
  • Satisfaction SCORE: measures individual client satisfaction.
  • Community SCORE: measures changes in groups or communities.

Each type of SCORE has different domains that can be used to report client outcomes. SCORE uses a 5-point rating scale – this scale varies for each type of SCORE
