Definition
A count of distinct TEI individual clients who have either Circumstances and/or Goals SCORE assessed in the reporting period. Each individual client with one or more Paired SCOREs (both Goals and Circumstances) recorded will only be counted once.
Clients will be unique within the service types even if that particular service types have been accessed multiple times.
The proportion of clients assessed is calculated out of all clients in the TEI program or each program activity and service type depending on the breakdown level.
Rationale for inclusion: As per the TEI Data Collection and Reporting guide, funded services should record Circumstances and/or Goals SCORE for at least 50% of their individual clients. This measure will track against the set target and will explore the supports required by TEI service providers to improve an understanding on how to measure client outcomes and how to use SCORE.
